Job Purpose: The Peer/Community Support Facilitator provides person-centred and flexible supports to persons with disabilities in community programming. This includes working directly with individuals with a developmental disability, physical disability, challenging behaviours and/or dual diagnosis in assisting to set and meet goals, make healthy life choices and promote rights and responsibilities. The position is a hands-on approach to plan, lead, and implement programs and activities that promote learning in daily living, social, recreational and life skills.
- Attention to detail and ability to complete tasks thoroughly
- Competent in working with persons with disabilities
- Reliable, responsible and dependable in fulfilling obligations
- Excellent verbal, interpersonal and written communication skills
- Ability to organize, plan and prioritize work with minimal supervision
- Ability to follow direction and work as a member of a team
- Competent computer skills in using Microsoft Office, Internet and email software, including Word, ability to learn custom database
- Ability to problem solve and make decisions accordingly
- Awareness and ability to make community connections
- An understanding and acceptance of the Independent Living philosophy i.e.: respecting a consumer’s right to take control of their own lives; examine options, make choices, take risks and make mistakes
- Diploma/degree in therapeutic recreation, social services, or related human service/ health professional degree
- Experience working with people with developmental disabilities/dual diagnosis is an asset
- Ability to work with large groups of people with a wide variety of disabilities requiring various levels of support
- Reliability, responsibility and accountability in fulfilling obligations to both the person(s) with disabilities and the organization
- Experience developing individualized person-centred plans an asset
